The procedure starts with anesthesia that numbs the wrist and hand for surgery. For the traditional open method, an insertion of about 2 inches is made on the wrist. Surgical instruments are then used to cut and enlarge the carpal tunnel.

For the endoscopic method, the cuts are quite small and one is made on the palm and the other on the wrist. A camera attached to a thin tube is then inserted into the carpal tunnel. Other instruments are then inserted into the carpal tunnel to cut the carpal ligament that is pressing the median nerve, making space for the median nerve and the tendons that are passing through the tunnel. This helps in reducing pain and improving function. The incisions are then stitched, followed with bandage to restrict the movement of hand and wrist.
